Gout - Medicines That Are Very Helpful

The medicines that are commonly used in alleviating the pain and other gout symptoms include:

- Adrenocorticotrophic hormone (ACTH)

- Allopurinol

- Colchicines

- Corticosteroids

- Non-steroidal anti-inflammatory drugs (NSAIDS)

- Probenicid

- Sulfinpyrazone

The first medication that is commonly prescribed in the acute cases of gout is the NSAIDS. The most effective one is Indomethacin. These drugs are initially prescribed in the highest dosage, and as symptoms subside the dosage is gradually reduced. The medicine needs to be continued until the pain and the inflammation vanish and remain so for over 48 hours. NSAIDS may have toxic effects if used for long, but when administered short term, they are well tolerated.

Allopurinol (marketed under the brand name Zyloprim) is a prescription medicine that functions in allaying by preventing the synthesis of uric acid in the body. It is very beneficial for chronic gout or gouty arthritis. The use is more preventive in nature, and it is not administered if the gout attack sets in. side effects include skin eruptions, inflammation of the blood vessel, and liver toxicity. Alterations in the enzyme profile of the liver, as for instance, transient increase in the level of alkaline phospahatase, have been reported. In other patients, incidences of hepatomegaly, necrosis of liver, hepatitis and jaundice have been known to occur.

Colchicine is a prescription medicine used to assuage acute gout attacks and to prevent recurrence of attacks as well. However it does not lower the levels of uric acid in the body. It provides relief by reducing inflammation. Serious side effect have been reported that include various stomach troubles like cramps, nausea, diarrhea, bone marrow problems, muscular inflammations, severe anemia, and serious reduction in the count of WBC, increasing the risk of secondary infections. Excessive high doses can prove to be fatal. Patients with reduced kidney function should avoid colchicines.

Patients who are intolerant to NSAIDS or Colchicine can be treated with ACTH or Corticosteroids. Patients suffering from acute gout take daily doses of Prednisone for 1 t 4 days, and then the dosage is lessened over the period of one or two weeks. ACTH is injected intramuscularly.

Probenicid is sold under the brand names Benemid or Probalan. It is prescribed for chronic gout and to prevent attacks. It is a uricosuric agent that accts on the kidney and aids in the elimination of uric acid from the system.

Sulfinpyrazone, sold under the brand name Anturane is another uricosuric agent that works by reducing the amount of uric acid in the blood, thereby preventing gout attacks.

Antiviral Drugs For Herpes Treatment

The major drugs developed to work against herpes simplex virus (HSV) are antiviral agents called nucleosides and nucleotide analogues, which block viral reproduction. They include acyclovir (Zovirax), valacyclovir (Valtrex), famciclovir (Famvir).

The antiviral medications available in pill form have been specifically developed for the treatment of genital herpes. Antiviral medications may significantly lessen the severity of a primary outbreak and reduce the time it takes genital herpes outbreaks to heal. The medication also decreases the number of days of painful symptoms and for some people, the number of days you can spread the virus. Antiviral medication is most effective if it is taken when you first notice the prodromal symptoms (tingling and pain) of a recurrent genital herpes outbreak and if they are taken for the next 5 to 7 days or until symptoms go away. Some people with frequent recurrent outbreaks (more than 6 recurrences a year) take antiviral medication every day (suppressive therapy) to help reduce the frequency and duration of recurrent outbreaks.

Brief History

Acyclovir (brand name Zovirax) is the oldest of the antiviral medications. It has been available since 1982 in a topical form (as an ointment) and sold since 1985 in pill form. Now acyclovir is available in a generic form.

Valtrex (valacyclovir) was the second antiviral medication to come to market in the United States, and it was approved by the FDA in December 15, 1995. Valtrex is manufactured by GlaxoSmithKline. Valtrex is a prodrug of acyclovir, meaning that the body converts it to acyclovir after it has been absorbed. This medication delivers acyclovir more efficiently so that the body absorbs much of the drug, which has the advantage of taking the medication fewer times during the day.

Famvir (famciclovir) is a prodrug manufactured by Novartis. When taken, the body converts it to the long-acting antiviral drug penciclovir. Like valacyclovir, it is well absorbed, persists for a longer time in the body, and can be taken less frequently than acyclovir.

Mechanism of action

Antiviral agents reduce viral replication by inhibiting viral DNA synthesis, needed to reproduce itself. This helps to keep the virus inactive.

Approved indications and uses

Acyclovir is indicated for:

• treatment of initial episodes and the management of recurrent episodes of genital herpes


• treatment of acute herpes zoster (shingles)


• treatment of chickenpox (varicella)

Valacyclovir is indicated for:

• treatment or suppression of genital herpes and for the suppression of recurrent genital herpes


• reduction of transmission of genital herpes


• treatment of herpes zoster


• treatment of cold sores (herpes labialis)

Famciclovir is indicated for:

• treatment or suppression of recurrent genital herpes


• treatment of acute herpes zoster

Efficacy

Clinical studies have not found any significant differences in effectiveness among the three medications. All are quite safe, very rarely producing any side effects at all. For the treatment of first genital herpes infections, oral acyclovir or valacyclovir is preferable to famciclovir. The efficacy of famciclovir for initial episode genital herpes infection has not been established.

Intravenous acyclovir is used to treat serious flare-ups or outbreaks that effect internal organs.

Adverse reactions and side effects

All these agents are well tolerated and have excellent safety records. Possible side effects from oral agents include nausea, vomiting, headache, fatigue, tremor, and very rarely, seizures. They can effect the kidney, however, and people with kidney problems should use them with caution and at lower doses. Intravenous administration increases the risk for kidney problems and can cause blood clots at the injection site. In rare cases, it can cause central nervous system complications.

Pharmacokinetics

Acyclovir has poor bioavailability of about 20% and a short half-life, which necessitates frequent dosing. Acyclovir speeds healing of lesions and suppress viral shedding if taken within 24 hours of the first indication of a recurrent episode. Early treatment may even prevent the development of lesions in some patients.

Valacyclovir provides a unique mechanism of enhancing the oral bioavailability of the parent compound, acyclovir. Valacyclovir is a prodrug converted to acyclovir in the intestine and liver. It has better bioavailability (about 55%) and has a longer duration of action than acyclovir. Thus, valacyclovir requires less frequent dosing than acyclovir. Valacyclovir is most effective if taken within 24 hours of the first signs of an outbreak.

Famciclovir is a prodrug for the active metabolite penciclovir. Famciclovir is converted into its active compound within the infected cell by contact with an enzyme from the virus. It has high bioavailability of 77%. It remains active in the body longer than acyclovir (half the dose is still active after 10 to 20 hours) and, like valacyclovir, requires less frequent dosing (usually two or three times a day). It is most effective if taken within six hours of onset of symptoms.

Both Famciclovir and valacyclovir have greater oral bioavailability that acyclovir. Famciclovir and valacyclovir, which are better absorbed by the body, can be taken less often than acyclovir. The simpler dosing schedule of famciclovir and valacyclovir, allows patients more convenient dosing regimens with less interruption of their activities of daily living.

What Kind of Eczema Treatment Can You Expect?

Eczema, a term used by most people to refer to Atopic Dermatitis, is an incurable disease which affects the skin. Although sure is not a possibility, there are things you can do to control the symptoms.

These things are a combination of medical attention and at home care. It is important for you to see a dermatologist when symptoms first appear as treating as early as possible helps you to keep your symptoms to a minimum and often makes the flare up period much shorter than it would be otherwise.

No matter how much you read about skin conditions, it is important that you understand a doctor diagnosis is important for you to plan the proper care for your condition. Treatment plans are also modified on an individual basis as you and your doctor know and understand more about triggering factors in your case.

First, if the dermatologist finds another condition, such as bacterial, viral, or fungal infections, he will prescribe a medicine to get rid of the infection. Antibiotics will be prescribed for bacterial infections, antiviral are prescribed for viral infections and antifungals will conquer fungal infection.

These may be applied to the skin, taken by mouth, or given in a shot, depending on the type as severity of the infection.

Treatment usually begins with topical medications which may or may not be coupled with antihistamines. Topical treatments are applied to the skin and are usually a cream which comes in a tube.

The newest form of topical medication is the topical Immunomodulators (TIMs). These do not have steroids in them, so they do not have the problems with thin skin that corticosteroids do. Elidel and Protopic are both TIMs.

Facts About Zoloft And Hot Flashes

Medical researchers have conducted a few studies concerning Zoloft and hot flashes. A few more studies have been done concerning Paxil and hot flashes. Twice as many published studies have focused on black cohosh and hot flashes and there are even more concerning this herb and other symptoms related to menopause. Why so many studies?

Traditional herbal remedies are often the subject of scientific scrutiny, because modern medicine would like to either “prove” or “disprove” their effectiveness. They would also like to know “why” botanical remedies are effective; so, many studies revolve around isolating the active component.

Researchers began studying antidepressants like Zoloft and hot flashes in 2002, around the same time that the Women’s Health Initiative released conclusions concerning the long-term health risks associated with hormone replacement therapy. Research concerning Paxil and hot flashes prior to 2002 focused on breast cancer survivors who are unable to use estrogen replacement therapy.

Scientists and researchers are unable to explain why these drugs may be effective, or even why they would be considered for use. It is likely that they were prescribed to women who were suffering from depression, which sometimes accompanies menopause. These women may have reported a reduction in hot flashes, leading doctors to suggest that they might be useful for controlling hot flashes.

Both Zoloft and Paxil belong to a group of drugs known as Selective Serotonin Reuptake Inhibitor or SSRI. These drugs are approved by the FDA to treat depression and some are approved for the treatment of premenstrual dysphoric disorder. They are not approved by the FDA to treat hot flashes, other symptoms related to menopause, nor are they approved to treat PMS, but doctors often prescribe them for these purposes.

Even though they are not approved by the FDA to relieve hot flashes, both the American College of Obstetricians and Gynecologists (ACOG) and the North American Menopause Society recommend that women with moderate to severe, menopause related hot flashes should consider an SSRI, if they cannot or choose not to take hormone replacement therapy. Interestingly, a brochure released by the ACOG mentions that herbs and botanicals are not approved by the FDA, but they never mention that SSRI drugs are not approved by the FDA to treat menopausal symptoms.

One study concerning Paxil and hot flashes experienced by breast cancer survivors is similar to a more recent study concerning the use of black cohosh. (Black cohosh is an herb used traditionally by Native American healers and passed down from generation to generation for the relief of hot flashes and other menopausal symptoms.) As previously mentioned, women who have had breast cancer are unable to take estrogen replacement therapy, in fact they must take a drug that limits the effects of estrogen for several years following surgery. Even in women who are not near menopause, this drug causes severe hot flashes.

In the study of Paxil and hot flashes, the antidepressant was shown to reduce hot flash frequency by as much as 79%. Black cohosh was shown to reduce hot flash frequency by as much as 100%. Of the 90 women who participated in the black cohosh study, none reported adverse side effects and no one dropped out. Of the 30 women who participated in the study of Paxil and hot flashes, three (10%) dropped out because of drowsiness and one dropped out because of anxiety, a possible adverse reaction to Paxil.

Recently research was conducted by the College of Medicine at the University of Arizona concerning Zoloft and hot flashes. A group of women aged 40-65, currently suffering from hot flashes, but not taking hormone replacement therapy, were recruited. The researchers used a number called the “hot flash score”, which is equal to the number of hot flashes a woman experiences multiplied by the numerical expression of their severity, to evaluate the effectiveness of the SSRI over a four week period. A similar study concerning black cohosh and hot flashes was conducted by the Mayo Clinic.

In the study of Zoloft and hot flashes, the average number of hot flashes the women experienced per week was 45. In the black cohosh trial, the average was 8 per day or 56 per week. Zoloft reduced the frequency of hot flashes by 5 per week or 11%. Black cohosh reduced the frequency by 28 per week or 50% and reduced the average “hot flash score” by 56%.

In the study of Zoloft and hot flashes there was no significant reduction in severity, but in their concluding statement the researchers say that “sertraline (the generic name for Zoloft) reduced the number of hot flashes and improved the hot flash score relative to placebo and may be an acceptable alternative treatment for women experiencing hot flashes”. So, these researchers believe that an 11% reduction in the hot flash score represents an effective alternative treatment. Numerous studies have shown that treatment with placebo can reduce hot flashes by 20-40%.

In the Zoloft and hot flashes study, 15 women dropped out, six because of adverse reactions to the drug, 9 without giving reason. None of the women dropped out of the black cohosh trial. No adverse events or unwanted side effects of any kind were reported. Women did note that their sleep improved, they were less tired and had less abnormal sweating.

The unwanted side effects related to the use of Zoloft include sleep disorders, weakness, dizziness, tremors, confusion, nausea, vomiting, decreased sex drive and inability to achieve orgasm. It can induce mood swings. At least one study has shown that it increased the risk of suicide in seniors, as it does in teens and pre-teens. In fact, the FDA has released a public health warning which states that “anyone currently using Zoloft for any reason has a greater chance of exhibiting suicidal thoughts or behaviors, regardless of age.”

Worldwide, 20% of all patients in clinical trials relating to Paxil dropped out due to unwanted side effects. The side effects are similar to those of Zoloft. Both drugs can cause increased sweating, which makes it even harder to understand why researchers would conduct studies concerning Zoloft and hot flashes, Paxil and hot flashes or any other drug that can cause increased sweating, since increased sweating is what frustrates women most about hot flashes and night sweats.

Over the years, hundreds and hundreds of studies have been conducted concerning the safety of black cohosh. No one knows how long native healers have used the herb. The only known side effect is stomach ache and this is an infrequent complaint. Recent scientific evaluations have shown that it does not increase the risk of breast or endometrial cancer. So, it is unclear why medical practitioners and societies would recommend something with as many side effects as Zoloft and Paxil, when there is a safer and more effective treatment.

Medication to Ease Your Hypertension

It seems that pharmaceutical companies are always introducing new products into their range to deal with ailments and conditions.

One such product that has been introduced to deal with hypertension or high blood pressure is Plendil. Whilst high blood pressure does not have a cure it can be maintained or controlled with calcium channel blockers like this.

Short Facts:

Most hypertension medications and calcium channel blockers work to relax arteries and veins in your blood vessels. Plendil is no different and operates on the same principle. When the arteries and veins are relaxed more oxygenated blood can flow to the heart which is of a higher quality. This is effective because the pressure that the heart was under is relieved and the heart is able to pump more effectively with less muscular stress.

The aim of the medication is to reduce serious cardiovascular disease to reduce the risk of death from the effects of high blood pressure.

Side Effects

This medication can produce some side effects which may cause you discomfort. One side effect is that you may experience dental problems, usually in your gums. You will need to increase your dental brushing and flossing routine to avoid these unfortunate side effects.

The other thing is that this medication does not react well with grapefruit. So, if you are a fan of grapefruits or grapefruit drinks you will need to stop your intake while on this course of medication. Unfortunately, when grapefruit and Plendil are mixed there can be dangerous side effects so it is not worth risking that morning drink. Your doctor should be able to advise you on what sort of beverages and meals are suitable for you while taking this drug.

Another thing is, that if you are used to crushing tablets up or chewing them to make it easier to get your daily dose you will not be able to do that while on your course of treatment.
